Responsibilities
- Assess home health and hospice patients in stable and emergent situations and document findings.
- Implement, develop, document, and evaluate the Plan of Care to ensure quality and continuity of care.
- Gather data on physical, psychosocial, and environmental factors and incorporate it into the Plan of Care.
- Maintain up-to-date patient records across visits, contacts, problems, plans, actions, and goals.
- Educate patients and families regarding care, medications, home safety, and self-care practices; tailor teaching to age, condition, culture.
- Supervise and provide clinical directions to HHA/STNA/LVN to ensure quality and continuity of service.
Requirements
- Current New York State license as a Registered Nurse; Required.
- Proof of current CPR certification; Required.
- 2+ years of home health nursing experience and 1–2 years in a clinical care setting; Required.
- OASIS experience; Required.
- Homecare Homebase software experience; Required.
- Valid driver’s license and reliable transportation; Required.
- ADN or Nursing Diploma required; BSN preferred.
- CHPN certification; Preferred.
