Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical, social and mental functions, meeting department objectives.
- Serve as co-leader in an interdisciplinary approach to patient groups.
- Assess patients, develop and implement individualized treatment plans, and monitor progress.
- Document progress and coordinate with the care team to optimize outcomes.
- Provide patient and family education on home exercise programs and safety.
Requirements
- Active physical therapy license in Oregon
- Experience in outpatient physical therapy (outpatient setting preferred)
- Ability to work effectively in a hospital-based outpatient setting
- Strong communication and collaboration skills
- Comfortable working as part of an interdisciplinary team
