Responsibilities
- Perform comprehensive PT evaluations and develop individualized plans of care for residents in a SNF setting.
- Implement evidence-based interventions focused on mobility, balance, strength, gait, and functional activities.
- Reassess residents regularly, adjust goals and treatments, and document progress and changes.
- Collaborate with an interdisciplinary team, participate in care conferences, and communicate care plans to staff and families.
- Maintain accurate, timely documentation of evaluations, treatment sessions, progress notes, and discharge planning.
- Educate residents, families, and care teams on safe mobility strategies, home exercise programs, and post-discharge maintenance.
Requirements
- Active Physical Therapist license in North Carolina or eligibility to practice there
- Ability to work in a skilled nursing facility setting with geriatric populations
- Availability to start on 08/31/2026 and commit to a 13-week assignment
- Willingness to work a 5x8 day schedule with 35 guaranteed hours per week
- Strong documentation and communication skills; ability to work collaboratively with a multidisciplinary team
