Responsibilities
- Provide direct patient care under supervision, including routine assessments and documentation
- Administer prescribed medications and maintain accurate medication records
- Monitor vital signs, observe for changes in patient condition, and escalate concerns per protocol
- Assist patients with activities of daily living and comfort measures
- Collaborate with the interdisciplinary team to implement care plans and ensure continuity of care
- Adhere to infection control practices, safety protocols, and patient safety standards
Requirements
- Active LPN license in good standing
- Current Basic Life Support (BLS) certification
- Experience in a skilled nursing or long-term care setting is preferred
- Diploma or equivalent from an approved LPN program
- Ability to work 12-hour daytime shifts in a local assignment
