Responsibilities
- Provide direct patient care to residents in a long-term care environment under supervision of RNs and physicians, including monitoring vital signs and daily needs.
- Administer prescribed medications and document administration accurately per facility policy.
- Assess resident status, report changes promptly to the supervising nurse, and participate in care planning.
- Assist with activities of daily living, mobility, and personal hygiene; promote resident dignity and comfort.
- Maintain accurate documentation in the charting system; ensure infection control and safety protocols are followed.
Requirements
- Current, active LPN license (state-specific) in good standing
- Ability to perform 12-hour shifts in a long-term care setting
- Strong communication and teamwork skills; ability to work with diverse resident populations
- Ability to meet standard pre-employment credentials and documentation requirements
