Responsibilities
- Perform comprehensive OT evaluations for residents in a long-term care setting.
- Develop and implement individualized treatment plans addressing activities of daily living, mobility, cognition, and safety.
- Monitor progress, document outcomes, and adjust plans in collaboration with the interdisciplinary team.
- Provide patient and family education on therapeutic exercises, adaptive equipment, and safety strategies.
- Maintain accurate records in the facility’s EMR and participate in care planning meetings with nursing and medical staff.
- Attend bi-weekly orientations on Mondays and comply with site onboarding and policies as required.
Requirements
- Vermont OT license required (may be pending)
- BLS certification required
- Prior travel experience required
- Ability to work in a long-term care setting
- 40 hours per week availability; schedule may include Monday–Sunday with bi-weekly orientations on Mondays
