Registered Nurse - Case Manager (Discharge Planning / Care Coordination) in an acute care hospital setting. Everett, WA, United States. Start date: May 18, 2026. Travel - Temporary Assignment. Schedule: 4x10 days. Weekly pay: $2,260–$2,343 per week. Guaranteed hours: 72 hours per 2-week period. This assignment places you on a Case Management / Care Coordination team that operates hospital-wide across two nearby campuses.

Responsibilities
- Perform discharge planning and care coordination for diverse patient populations across Med-Surg, LLOS, ED in an acute hospital setting
- Assess needs, develop and implement plans of care, coordinate DME and post-discharge services
- Collaborate with an interdisciplinary team including Social Services to support safe and timely dispositions
- Document plans and progress in EPIC; ensure accurate communication across units and campuses
- Provide coverage across hospital areas as needed, with ability to float between two campuses
Requirements
- Active RN licensure in Washington or pending license accepted at submittal
- BLS (Basic Life Support) certification required
- Minimum 1 year of acute care experience; required experience in discharge planning / care coordination
- First-timers accepted; diverse case management exposure (Med-Surg, LLOS, ED) preferred
- EPIC charting experience required
- Education: graduation from an approved nursing program; ability to practice as an RN in WA
- Ability to work in a hospital setting and to float across units and campuses
- References: minimum 2 supervisory references covering 1 year of employment within the last 3 years
- Background/Nursys: disclose any background or Nursys hits as part of the submittal
