- On-call rotation for off-shift, weekend, and holiday coverage
- On-site crisis intervention and outpatient counseling duties within a hospital-adjacent behavioral health program
Responsibilities
- Provide crisis intervention and behavioral health evaluations in emergency and outpatient settings
- Conduct comprehensive psychosocial assessments, including psychosocial history, risk assessment, substance use screening, and family and social supports
- Develop, implement, and document person-centered treatment plans with measurable goals
- Deliver individual and group counseling to clients aged 5 and older; coordinate care with the clinic team and other health professionals
- Maintain timely, accurate progress notes and documentation in the medical record; adjust treatment plans as needed
- Participate in on-call rotations to support Emergency Department mental health evaluations and coordinate with the broader care continuum
Requirements
- Master’s degree in Social Work; licensure as LMSW, LCSW, LMHC or LCAT required
- Must hold LMSW for on-call requirements
- Licensed to practice in New York and familiar with NYS OMH requirements; prior New York work history preferred
- Minimum 2 years of experience as an LCSW or LMSW
- Experience working with clients ages 5 and older
- BLS certification;Covid-19 and influenza vaccines required
- Ability to work in a hospital-based outpatient and crisis intervention setting; strong documentation and collaboration skills
