RN Case Manager (Discharge Planning / Care Coordination) – Everett, WA, United States – Start date 05/04/2026 – Duration: Weeks – Weekly pay $2,186–$2,278 per week – Shift: 5x8 Days – Setting: Hospital, Level II trauma center – Travel - temporary assignment

- Travel assignment with cross-campus coverage; open to qualified traveling RNs or MSWs
Responsibilities
- Provide discharge planning and care coordination across acute care settings to support timely and safe patient transitions
- Conduct comprehensive needs assessments, coordinate DME, home health, and post-discharge resources; collaborate with Social Services and interdisciplinary teams
- Manage a caseload of approximately 25 patients, across Med-Surg, low-level observation services (LLOS), and ED as needed
- Document plans of care, progress notes, and discharge summaries in EPIC; communicate updates to care teams and families
- Float to other units and campuses as required to ensure appropriate coverage and continuity of care
Requirements
- Licensure: Active RN license in Washington state or MSW with relevant case management experience
- Certification: BLS required
- Experience: Minimum 1 year of acute care experience with discharge planning / care coordination; diverse case management experience (Med-Surg, LLOS, ED) preferred
- Education: RN (BSN) or MSW; clinical care coordination experience required
- Skills: Proficient in EPIC or similar EHR; strong assessment, communication, and teamwork skills; ability to manage a high-volume caseload and complex discharges
- Ability to work in a hospital setting with the flexibility to float across units as needed; weekends on a rotating schedule
