Travel Social Worker Case Management Job
This 13-week travel assignment starts 06/29/2026 and is conducted in a Level II trauma center setting. Weekly pay ranges from $1,717 to $1,756, with 72 guaranteed hours per 2-week period. Shift is 4x10 days. The assignment is a temporary travel position managed through Healthforce Healthcare LLC.

Responsibilities
- Perform comprehensive discharge planning and care coordination for adult and geriatric inpatients; address psychosocial needs and barriers to discharge
- Conduct biopsychosocial assessments and document findings to guide care planning
- Develop and implement safe, timely discharge plans; coordinate referrals to home health, DME, rehabilitation, and long-term care as appropriate
- Collaborate with interdisciplinary teams (PT, interpretation services, social services, pharmacy, transportation) to support patient outcomes
- Manage a caseload of approximately 18–24 patients (higher on weekends) with typical daily census around 25
- Support high-risk/guardianship cases as part of diverse case management; float within the hospital and to sister campuses as needed
- Document in EPIC; ensure accurate charting and compliance with regulatory standards
- Participate in required weekend coverage (every other weekend) and holiday coverage as scheduled
Requirements
- Education: Master’s degree in Social Work required; LMSW or LCSW accepted; MSW candidates must have WA licensure in hand (pending licenses accepted for submittal)
- Licensure: Washington state social work license appropriate to the level (LICSW, LSWAIC, LASW, LSWAA accepted); licensure in hand for unit; pending license considered for submittal
- Certifications: BLS required
- Experience: Minimum 1 year of acute care social work; experience in discharge planning/care coordination; diverse case management experience (Med-Surg, LLOS, ED) preferred
- Location and logistics: Not accepting local applicants; ability to work in Everett, WA; float within hospital and to related campuses as needed
- Other: EPIC charting proficiency; willingness to participate in orientation (40 hours) and pre-start modules (~16 hours); weekend and holiday commitments as noted
