Travel Social Worker Case Management Job
Social Worker – Case Management, Everett, Washington. Start date: 06/29/2026. Contract: 13 weeks. Weekly pay: $1,714–$1,754 per week. Guaranteed hours: 72 hours per 2-week pay period. Shift: 4x10 Days. Work setting: inpatient, Level II Trauma Center with adult and geriatric patients. Responsibilities
- Perform discharge planning and care coordination for adult and geriatric inpatients; identify barriers to safe transitions and arrange appropriate community and post-acute resources.
- Conduct psychosocial assessments; develop and implement comprehensive care plans in collaboration with patients, families, and the interdisciplinary team.
- Document care plans, progress notes, and relevant clinical information in EPIC; ensure accurate care planning and communication across teams.
- Facilitate smooth transitions to home or alternate care settings; coordinate with PT, interpretation services, social services, pharmacy, and transportation to support patient goals.
- Participate in rounds and care conferences; manage complex or high-risk cases; escalate issues per protocol.
- Float within the hospital and between sister campuses as needed to cover patient care and program needs.
Requirements
- Licensure: Active WA license appropriate to the role (MSW track: WA LICSW/LSWAIC/LASW/LSWAA; RN track: WA RN or WA Compact) in-hand
- Certifications: BLS (for RN track)
- Experience: Minimum 1 year of clinical experience; acute care discharge planning / care coordination
- Clinical exposure: Inpatient experience with adults and geriatrics; diverse case management across Med‑Surg, long length of stay, and ED preferred
- Skills: EPIC charting experience; strong discharge planning, documentation, and interprofessional communication
- Education: MSW degree for MSW track or nursing degree for RN track, aligned with licensure
- Availability: Schedule is 4x10 days; weekends every other weekend; holiday coverage required (3 of 5 major holidays)
