Responsibilities
- Assist with and supervise the day-to-day nursing activities during your shift, ensuring care aligns with current standards and regulations.
- Administer prescribed medications and treatments in accordance with physician orders and facility policy; monitor for adverse reactions.
- Perform resident assessments, document findings, and communicate changes to the nursing team to support care planning.
- Implement and support rehabilitative and restorative nursing practices as part of residents’ care plans.
- Collaborate with the charge nurse and other care staff to maintain a safe, quality care environment; monitor resident safety and respond to changes in condition.
- Complete timely and accurate charting and documentation; support continuity of care across shifts.
Requirements
- Active LPN license in the state of Colorado.
- Minimum 3 years of experience in a hospital, skilled nursing facility, or comparable setting.
- Training in rehabilitative and restorative nursing practices.
- IV certification preferred for LPNs; current BLS certification.
- CAPS check (Colorado adult protective services) required.
- Ability to work in a long-term care memory care environment.
- Reliable transportation available due to rural Gunnison location (no public transportation).
