Responsibilities
- Conduct comprehensive evaluations and identify equipment needs related to speech and language function.
- Develop and implement a Plan of Care to improve functional communication and, when appropriate, swallowing function.
- Administer speech-language pathology interventions using specialized techniques and equipment.
- Recommend communication devices or aids as indicated to support patient function.
- Confer with referring physician and interdisciplinary team to ensure coordinated, comprehensive care within the Plan of Care.
- Document patient care, including progress, changes to the POC, and continuing need for therapy; provide patient and family/caregiver education on tasks and resources for continued independence.
- Identify needs for additional services and refer to appropriate resources; may supervise home health aide in procedures tied to the POC.
- Participate in case conferences, in-services, and other meetings required to ensure coordinated care; perform other related duties as assigned.

Requirements
- Current Certificate of Clinical Competence (CCC) in Speech-Language Pathology from ASHA, or CCC-eligible, and current state licensure as applicable.
- Proof of current CPR certification.
- Current driver's license and reliable transportation; must maintain appropriate auto insurance.
- 1 year of clinical experience in Speech-Language Pathology.
- Education: Master's degree in Speech-Language Pathology from a program accredited by the Educational Standards Board or meeting ASHA certification requirements.
- Experience in a home care environment preferred.
