Responsibilities
- Assess residents’ physical therapy needs and develop individualized treatment plans within the SNF setting
- Implement therapeutic interventions to improve mobility, strength, balance, and functional independence
- Provide gait training, therapeutic exercises, manual therapy, and assistive device instruction as indicated
- Reassess progress regularly, adjust plans, and document outcomes in electronic health records
- Collaborate with an interdisciplinary team to optimize care and discharge planning
- Maintain safety, infection control, and patient-care standards; educate patients and families as appropriate
Requirements
- Doctor of Physical Therapy (DPT) degree or equivalent
- Active Maryland Physical Therapy license in good standing (or eligibility)
- Ability to work effectively in a skilled nursing facility environment
- Experience in geriatric rehabilitation or SNF settings preferred
- Strong communication, assessment, and documentation skills
