Responsibilities
- Actively monitor and amend patient visit schedules to maximize productivity and address patient needs, including rescheduling and load balancing; communicate changes to supervision as needed.
- Perform comprehensive PT assessments for assigned patients, incorporating psychosocial, educational, safety, and risk factor parameters in line with regulations and APTA guidelines.
- Develop and document a plan of care to promote functional independence and safety, with patient-specific goals and logical interventions advancing toward those goals.
- Plan and provide skilled PT treatment, including therapeutic techniques and patient/caregiver education; assess patient status at each visit and document progress.
- Coordinate patient care as a team member with physicians, nurses, caregivers, and other rehab services, ensuring clear communication and adherence to policies and standards.
- Supervise and direct clinical activities of PTAs, PT students, and home care aides in accordance with regulations and department policies; ensure appropriate delegation and support.
Requirements
- Current physical therapist license to practice in Vermont.
- Ability to supervise PTAs, PT students, and home care aides.
- Experience with, or strong familiarity of, conducting OASIS time points and related documentation.
- Proficiency with required paperwork and billing processes; ability to complete forms (e.g., HHABN, Medicare notices) accurately and timely.
- Strong assessment, plan-of-care development, and patient education skills; capacity to work in a team-based, skilled nursing setting.
- Available for a travel assignment starting 05/04/2026 and lasting approximately 13 weeks; 37 hours per week.
