Responsibilities
- Evaluate residents for physical therapy needs and develop individualized plans of care aimed at improving mobility, strength, balance, and functional independence.
- Implement evidence-based PT interventions, including gait training, ROM exercises, endurance activities, and task-specific training within a skilled nursing setting.
- Assess and monitor progress, document outcomes, and adjust treatment plans in collaboration with the care team.
- Educate residents and families on home exercise programs, safety, fall prevention, and activity modification to support continued progress.
- Communicate clearly with nurses, physicians, and other therapists to ensure cohesive, patient-centered care and timely discharge planning.
- Maintain accurate documentation in the electronic health record and ensure compliance with regulatory and facility standards.
Requirements
- Active Physical Therapist license (state of Maine or eligible for licensure by endorsement)
- Ability to work in a skilled nursing facility setting and collaborate within an interdisciplinary team
- Experience in geriatrics or SNF rehabilitation preferred
- Strong documentation skills and proficiency with electronic health records
- Effective communication and teamwork abilities
- Availability for a 13-week travel assignment with guaranteed hours
