Responsibilities
- Provide direct nursing care to residents in a long-term care setting
- Administer prescribed medications and monitor for adverse reactions per physician orders
- Monitor vital signs, assess changes, and escalate to the supervising clinician as needed
- Assist residents with activities of daily living, mobility, and comfort measures
- Maintain accurate documentation in the charting system per facility policy
- Collaborate with the interdisciplinary team to implement care plans and ensure safety and quality
Requirements
- Active LPN license in Vermont
- Availability for day or evening shifts as assigned
- Ability to work effectively in a long-term care environment
- Strong communication, observation, and documentation skills
- Flexibility to adapt to different units and shift patterns during the assignment
- Willingness to travel for a temporary assignment
