Responsibilities
- Perform clinical assessments to identify patient needs and appropriateness of physical therapy
- Develop, implement, and review a Plan of Care (POC); document progress notes and other clinical records; revise the POC as needed
- Plan and provide treatment for neuromuscular, neurological, and orthopedic disorders based on referrals and evaluations
- Instruct patients and families on transfer techniques and safe use of equipment (walkers, wheelchairs, canes, braces, prosthetic devices); arrange appropriate rehabilitative equipment as needed
- Educate patients and families about therapeutic procedures, considering age, condition, culture, and learning readiness
- Collaborate with an interdisciplinary team and supervise other staff when applicable; apply infection control and universal precautions
Requirements
- Current/state license as a Physical Therapist – Required
- CPR certification – Required
- Current driver’s license with reliable transportation and insurance – Required
- 1–2 years of experience as a Physical Therapist in an acute care or rehabilitation setting – Required
- Previous home health/home care experience – Preferred
- Education: Bachelor of Science in Physical Therapy from an APTA-approved program – Required
