Responsibilities
- Conduct initial and periodic OT evaluations for residents with acute care needs and translate findings into individualized treatment plans with measurable short-term goals.
- Deliver evidence-based interventions to improve activities of daily living, functional mobility, cognition, balance, safety, and independence.
- Document sessions, progress, and discharge planning in the electronic health record; communicate recommendations during rounds and with families as appropriate.
- Collaborate with nursing, physical therapy, speech-language pathology, and medical staff to optimize plans of care and coordinate seamless transitions of care.
- Educate residents and family members on home exercise programs, adaptive equipment, safety strategies, and activity modifications to sustain gains after discharge.
- Ensure compliance with licensure requirements, facility policies, and payer guidelines; participate in quality improvement and patient care initiatives as needed.

Requirements
- Active license to practice Occupational Therapy in California (or eligibility)
- Degree in Occupational Therapy from an accredited program (e.g., MSOT, MOT, or OTD)
- Minimum 1 year of clinical OT experience; acute care or SNF experience preferred
- Strong assessment, goal-setting, and documentation skills; proficient with electronic health records
- Ability to work in a fast-paced skilled nursing facility environment and collaborate with an interdisciplinary team
- Availability for the 13-week assignment starting 08/25/2026 and the defined shift
