Responsibilities
- Perform OT evaluations and develop individualized treatment plans for residents with functional limitations.
- Implement therapy sessions targeting activities of daily living, mobility, and cognitive/perceptual skills as appropriate.
- Reassess and document progress regularly; adjust plans based on outcomes and payer guidelines.
- Collaborate with an interdisciplinary team (PT, nursing, physicians, family) to coordinate care and support discharge planning.
- Document in the facility’s electronic medical record and ensure compliance with applicable regulations and standards.
- Provide patient and family education on home programs, equipment use, and strategies to maintain progress after discharge.
Requirements
- Active OT license in Montana or eligibility to obtain MT licensure.
- Master’s degree in Occupational Therapy from an accredited program.
- Ability to work in a skilled nursing facility environment with a focus on acute care needs.
- Strong communication and documentation skills; proficient with EMR systems.
- Availability to work the Day shift and willingness to undertake a 13-week travel assignment.
- Prior OT experience in SNF or acute care setting preferred.
