Responsibilities
- Provide direct patient care within a skilled nursing facility, under supervision as applicable
- Administer medications and document in patient records according to care plans and facility policy
- Monitor vital signs, assess patient condition, and report changes promptly to the supervising nurse
- Assist with activities of daily living (ADLs) and support patient mobility and safety
- Maintain accurate, up-to-date charting and documentation in the electronic medical record
- Collaborate with the healthcare team to support individualized care plans and ensure quality of care
Requirements
- Active LPN license in Wisconsin
- Ability to work in a skilled nursing facility setting
- Availability for a 13-week travel assignment with guaranteed 40 hours per week
- Willingness to work the Weekend Warrior schedule (Fri or Mon 8 hours; Sat/Sun 16 hours)
