Responsibilities
- Deliver coordinated nursing care for a patient or assigned group of patients according to established standards of care and the nursing process.
- Assess, plan, implement, and evaluate patient care, ensuring safety and quality at all times.
- Supervise and direct activities of various levels of assigned nursing staff; coordinate care with other disciplines using critical thinking and independent judgment.
- Collaborate with the multidisciplinary team to develop and modify the plan of care based on patient response and changing needs.
- For Float Pool assignments, clock in to the unit being worked and adhere to unit-specific education and non-productive hours requirements.
Requirements
- Ability to provide nursing care in a home healthcare - hospice setting.
- Strong assessment, critical thinking, and clinical judgment skills.
- Effective communication with patients, families, and the care team; collaboration across disciplines.
- Willingness to work 12-hour day shifts and adapt to unit staffing needs.
- Demonstrated professionalism and reliability in a travel assignment context.
- Ability to adhere to unit policies for Float Pool work tasks and documentation.
