Responsibilities
- Provide direct patient care to residents per individualized care plans
- Administer prescribed medications and document administration accurately
- Monitor vital signs, assess changes in resident condition, and report to supervising nurses
- Assist with activities of daily living and rehabilitative tasks as appropriate
- Maintain charting and documentation for shifts, treatments, and care plans
- Collaborate with licensed staff, interdisciplinary team, and family members to support resident well-being
- Ensure infection control practices and safe handling of medications and equipment
- Respond to call lights and emergencies in a timely, professional manner

Requirements
- Active LPN license in the state of New Mexico (or eligibility to practice there)
- LPN diploma or equivalent
- CPR/BLS certification
- 1 year of licensed practical nurse experience, preferably in long-term care or skilled nursing
- Ability to work 12-hour night shifts and follow resident care plans
- Strong communication, teamwork, and documentation skills
