Responsibilities
- Provide direct nursing care to residents per individualized care plans.
- Administer medications and treatments as prescribed, following facility policy.
- Monitor resident status, perform routine assessments, and document findings in the chart.
- Assist residents with activities of daily living and mobility support.
- Communicate changes in resident condition to the supervising nurse and participate in care planning and shift handoffs.
Requirements
- Active LPN license in good standing.
- Availability to work the assigned nights and days: Wednesdays, Thursdays, Fridays, Saturdays, and every other Monday, with 40 guaranteed hours per week.
- Prior long-term care or skilled nursing experience preferred.
- Strong clinical assessment, documentation, and communication skills.
- BLS certification preferred.
