Responsibilities
- Conduct comprehensive PT evaluations and develop individualized treatment plans for residents.
- Deliver therapy interventions to improve mobility, strength, balance, gait, and daily function; monitor progress and adjust plans as needed.
- Reassess residents regularly, document progress toward goals, and communicate changes to the care team.
- Collaborate with nursing, therapy aides, physicians, and other disciplines to coordinate care and optimize rehabilitation outcomes.
- Educate residents and families on exercises, home programs, safety strategies, and discharge planning; support smooth transitions of care.
- Maintain accurate, timely documentation and ensure compliance with facility policies, regulatory requirements, and infection control practices.
Requirements
- Doctor of Physical Therapy (DPT) degree from an accredited program.
- Current, valid physical therapy license to practice in the United States (or eligibility to obtain).
- Basic Life Support (BLS) or CPR certification.
- Experience in skilled nursing or long-term care settings preferred.
- Ability to commit to a 13-week travel assignment with guaranteed 38 hours per week.
- Availability to work the scheduled shift: 08:00–16:30, five days per week.
