Responsibilities
- Coordinate and manage comprehensive care for a geriatric patient population from pre-admission through post-discharge.
- Lead interdisciplinary care planning, ensuring the care plan aligns with the patient’s clinical course and ongoing needs.
- Collaborate with physicians, nurses, social workers, and other members of the care team to optimize outcomes and appropriate resource utilization.
- Monitor diagnostic testing, treatments, and procedures; communicate with the care team about plan adjustments and discharge planning.
- Focus on complex cases, including frequent ED utilizers, chronic pain, substance use, and housing challenges; facilitate smooth transitions of care.
- Support transfers to tertiary centers when needed, ensuring required information is provided for continuity of treatment.
Requirements
- Licenses/Certifications: Registered Nurse (RN) license in the state of practice (California) required
- Certifications: Basic Life Support (BLS) certification from an approved vendor required
- Education: Bachelor’s Degree in Nursing (BSN) preferred
- Experience: At least two years of recent nursing experience; care management experience preferred
- Specialization/Setting: Ability to work effectively in an acute care, hospital-based environment
- Additional: Case management certification preferred
