Physical Therapist – Home Health, Sunnyvale, CA. Travel temporary assignment starting 06/15/2026 for 13 weeks. Weekly pay range: $2,454–$2,648 per week with guaranteed hours of 40. Shift: 5x8 Days (8:00 am–5:30 pm; schedule can be flexible based on patient availability). Setting includes home health and hospice with services delivered across the San Francisco Bay Area and nearby communities.

Responsibilities
- Evaluate and treat patients using medically prescribed physical therapy programs.
- Develop and implement individualized treatment plans to restore function and maximize independence.
- Supervise Physical Therapy Assistants as assigned.
- Perform initial admission/evaluation visits and establish goals and discharge plans.
- Document observations, clinical findings, problems, interventions, goals, and discharge plans.
- Notify the Clinical Team Manager of OASIS scores within established timelines.
Requirements
- One year of Home Health experience
- Ability to perform initial admissions/evaluations and deliver skilled PT interventions in a home health/hospice setting
- Willingness to travel within the San Francisco Bay Area territory (including SF, Brisbane, Daly City, Colma, South SF, Pacifica, Millbrae, Hillsborough, Burlingame, San Mateo and nearby areas)
- Strong documentation skills and ability to meet OASIS timelines
- Effective communication and collaboration with patients, families, and the clinical team
- Ability to work independently in patient homes and manage a variable schedule
