Travel Licensed Practical Nurse Long Term Care Job
Licensed Practical Nurse (LPN) – Long Term Care in Milford, New Hampshire. Start date: 08/01/2026. 8-week travel assignment with guaranteed 40 hours per week. Evening shift (3:00 PM–11:30 PM), 5 days per week. Travel position in a skilled nursing facility, employed through Healthforce Healthcare LLC. Weekly pay ranges from $1,646 to $1,746.

Responsibilities
- Provide direct nursing care to residents, including assistance with activities of daily living and restorative nursing support.
- Monitor residents, document vital signs, and report notable changes to the supervising RN.
- Administer prescribed medications and document administration in accordance with policy and procedures.
- Perform routine procedures such as wound care, dressing changes, and basic catheter care as directed.
- Support safe resident mobility, transfers, and fall prevention; assist with daily routines and comfort measures.
- Collaborate with the interdisciplinary team to update care plans and maintain accurate, timely charting.
Requirements
- Active Licensed Practical Nurse license in New Hampshire or eligibility for licensure in NH.
- Current CPR/BLS certification.
- Experience in long-term care or skilled nursing settings preferred.
- Ability to work evenings in a fast-paced clinical environment.
- Strong communication and documentation skills.
- Completion of an approved practical nursing program (LPN diploma).
