Responsibilities
- Conduct initial and ongoing PT evaluations for residents; develop and update individualized plans of care.
- Implement evidence-based interventions to improve mobility, strength, balance, gait, and functional independence.
- Monitor progress, adjust plans of care, and document daily progress and outcomes in resident records per regulatory standards.
- Collaborate with an interdisciplinary team (nurses, therapy colleagues, case managers) to optimize care and discharge planning.
- Educate residents and families on home exercise programs, safety strategies, fall prevention, and use of assistive devices.
- Ensure safe transfers, positioning, and therapeutic activities; maintain adherence to infection control and documentation requirements.
Requirements
- Active Physical Therapist license in the United States (state licensure as appropriate)
- Ability to work effectively in a skilled nursing facility setting; experience with geriatric populations preferred
- Strong communication and documentation skills; proven ability to collaborate with a multidisciplinary team
- Availability to commit to a 14-week travel assignment with 40 hours per week (on-site)
- This position requires on-site presence in Unity, NH; travel readiness and reliability
