Responsibilities
- Assess home health and hospice patients in stable and emergent situations, document findings, and communicate changes to the interdisciplinary team to modify the Plan of Care as needed.
- Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care, including updating goals and interventions based on patient response and payer requirements.
- Gather comprehensive data on physical, psychological, social, spiritual, and cultural factors affecting health; integrate findings into the POC and care plan to support patient-centered outcomes.
- Maintain up-to-date patient records, ensuring visits, contacts, problems, plans, actions, and goals are accurately stated and changes are reflected in the medical record as they occur.
- Apply infection control measures in home settings to protect both patient and staff; provide patient and family education on infection prevention and home safety.
- Educate patients and families about care tasks, medications, diet, and safety; use developmentally appropriate teaching methods and promote self-care practices; supervise HHA/STNA/LVN as needed to ensure quality of service.
Requirements
- Current North Carolina RN license (active)
- CPR certification (Required)
- CHPN certification preferred
- 1-2 years RN clinical care experience; 2+ years home health experience
- ADN or Nursing Diploma required; BSN preferred
- Proficiency with Homecare Homebase software and OASIS data collection/documentation required
- Valid driver's license and reliable transportation; ability to travel locally to patient residences
