Licensed Practical Nurse (LPN) – Long Term Care | Post-Acute Skilled Nursing Rehab | Middlebury, VT
Start date: 08/13/2026 | Duration: 4 weeks | Shift: 3x12 Nights | Weekly pay: $1,389-$1,432 per week | Guaranteed hours: 0 | Travel - Temporary Assignment in a 98-bed post-acute care unit.

Responsibilities
- Provide direct nursing care to post-acute and long-term residents within scope of practice for an LPN.
- Administer medications and therapies per physician orders; monitor for adverse reactions and document in the EPIC EMR.
- Perform phlebotomy, EKGs, and assist with transports as needed; support nebulizer to inhaler treatments and starting IVs where certified.
- Assess patient status, monitor vitals, wound care, and ADLs; report changes to the charge nurse and care team.
- Collaborate with the Unit Leader/Director of Nursing and interdisciplinary team to optimize care plans; participate in rounds and care conferences.
- Float to other units as required to support patient care needs; adhere to infection control and safety protocols.
Requirements
- Active LPN license on file at submission; appropriate licensure to practice in Vermont.
- BLS certification required.
- EPIC EMR experience within the past 2 years required.
- IV certification preferred.
- Dementia-capable care certification preferred.
- Minimum 1 year of clinical experience; 1+ year in a post-acute/skilled nursing rehab setting preferred.
- Prior travel experience required.
- Floating ability; willingness to work across units as needed.
- Must be able to work all holidays; schedule includes holiday coverage.
- Reliable transportation required; public transportation not available in the area.
