Responsibilities
- Provide rehabilitation services as a Speech Language Pathologist in a SNF/ICF setting
- Perform evaluations, reassessments, treatments, and other clinical services aligned with licensure and scope of practice
- Develop, implement, and carry out individualized treatment plans in collaboration with supervising therapist, physician, and interdisciplinary team
- Apply evidence-based therapeutic interventions to maximize communication, swallowing, mobility, function, and quality of life
- Educate residents, families, and caregivers regarding treatment plans and patient progress
- Complete accurate, timely, and compliant documentation; document progress, changes in plan of care, and discharges per facility, state, federal, and payer requirements
- Supervise therapy assistants when applicable to the role
- Be available to work Hawaii State and Federal Holidays as necessary; possible floating to other units or facilities based on census and staffing needs
- Expect to participate in orientation to the rehabilitation department, EMR documentation (Net Health Rehab Optima, Epic), and facility workflows
- Must arrive the Thursday before start to complete the employee health process
Requirements
- Current Hawaii State license in good standing appropriate to the discipline; active by start date
- Current Basic Life Support (BLS) certification
- Minimum two (2) years of SNF/ICF experience preferred
- Ability to perform duties within applicable state law, professional practice standards, and facility workflows
- Willingness to work a rotating schedule, including one weekend day as needed
- Eligibility to work in the U.S. and travel assignment requirements; must comply with Hawaii licensure rules
