Responsibilities
- Administer medications and treatments per physician orders and care plans
- Conduct daily nursing assessments, monitor vital signs, and document findings
- Implement and adjust care plans in collaboration with the interdisciplinary care team
- Provide LTC-focused nursing care and assist with activities of daily living as needed
- Maintain accurate PCC-related documentation and ensure resident charts reflect current status
- Communicate changes in resident condition promptly to the charge nurse and attending physician
Requirements
- Must have prior experience in long-term care (LTC) and PCC-related tasks
- Must be able to hit the ground running; extensive orientation will not be accommodated
- Willingness to work a 3x12-hour day shift schedule starting 10/01/2026
- Ability to work in a fast-paced LTC setting and adhere to facility protocols
- Strong communication and documentation skills
