Responsibilities
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse.
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and Interdisciplinary Team.
- Contribute to an age-appropriate, culturally relevant plan of resident care, involving the resident/family and healthcare team; monitor and support progress toward desired outcomes.
- Reassess resident status and recommend revisions to the plan of care; escalate referrals as needed to ensure comprehensive care.
- Maintain current knowledge and competency in nursing practice; participate in mentoring or orientation of staff; adhere to safety and infection control policies.
- Collaborate with nursing staff, physicians, and ancillary staff to ensure continuity of care and patient safety for adult and geriatric residents.
Requirements
- Active New York LPN license
- BLS (AHA or RQI) certification
- Minimum 1 year of LPN experience in a long-term care setting
- Ability to provide care for adult and geriatric residents
- Demonstrated ability to work within a team and follow policy under supervision of an RN
- Willingness to work a night shift in a travel assignment setting
