Responsibilities
- Provide direct patient care for ICU-level patients in a home hospice setting, delivering comprehensive assessment and monitoring.
- Administer prescribed medications and therapies, manage IV therapies, and respond to changes in patient condition with appropriate interventions.
- Monitor vital signs, track symptoms, and coordinate with the interdisciplinary team to adjust care plans as needed.
- Document care, interventions, and patient responses in Cerner and Epic electronic medical records; ensure accurate and timely charting.
- Educate patients and family caregivers on symptom management, safety, and end-of-life care planning; support families through the care trajectory.
- Adhere to clinical policies and safety protocols (including HLFS requirements) and maintain high standards of infection control and patient safety.
Requirements
- Active registered nursing license in California
- BLS and ACLS certifications; Hospital Fire and Life Safety (HLFS) training
- Cerner and Epic experience within the last 5 years; Epic test-out option available
- Must have been an employee of an organization or have successfully completed a contract within the last 5 years
- Ability to work in a home healthcare - hospice setting and travel as required
- Start date readiness for the assignment beginning 08/17/2026
