Responsibilities
- Provide direct bedside nursing care to residents per existing care plans and physician orders.
- Monitor vital signs, assess changes in condition, and administer medications and treatments within scope of practice; document accurately in the Point-Click-Care system.
- Collaborate with the interdisciplinary team to update care plans and communicate changes in resident status.
- Assist residents with activities of daily living, promote safety, mobility, and independence while maintaining resident rights and privacy.
- Ensure accurate charting, reporting, and adherence to infection control and safety protocols.

Requirements
- Active Licensed Practical Nurse license in good standing (West Virginia)
- Point-Click-Care (PCC) experience required
- Experience in long-term care or LTC setting preferred
- Ability to work night shift (7:00 PM–7:30 AM) and participate in a 2-week rotation schedule
- Must be willing to travel and comply with the radius requirement: traveler cannot be more than 100 miles from the facility
