Responsibilities
- Assess and treat residents with physical therapy needs to improve mobility, strength, balance, and functional independence
- Develop and implement individualized plans of care in collaboration with the interdisciplinary team
- Administer evidence-based therapies and modalities appropriate for geriatric rehabilitation
- Document evaluated findings, treatment plans, progress, and discharge summaries in the medical record
- Monitor patient response to treatment and adjust plans as needed
- Educate residents and caregivers on home exercise programs, safety, and fall-prevention strategies
- Ensure patient safety, infection control, and adherence to facility policies and regulations
Requirements
- Active physical therapy license in good standing (state-specific licensure; Mississippi license preferred)
- Doctoral degree in Physical Therapy (DPT) or equivalent
- Experience in skilled nursing facility or geriatric rehabilitation preferred
- Ability to work in a travel assignment and adapt to a SNF setting
- Strong communication and documentation skills
