Responsibilities
- Assess residents’ physical therapy needs and develop individualized treatment plans.
- Implement therapeutic exercises, gait and balance training, ROM, strength, and functional mobility programs.
- Apply therapeutic modalities and document responses to treatment on an ongoing basis.
- Monitor progress, adjust treatment plans, and communicate outcomes to the care team.
- Support discharge planning and coordinate with other disciplines to optimize patient outcomes.
- Maintain accurate and timely documentation of evaluations, plans, progress notes, and outcomes.
Requirements
- Current, unrestricted Physical Therapist license in New Hampshire (or eligible for licensure in NH)
- Doctor of Physical Therapy (DPT) degree or equivalent
- Experience in geriatric rehab or skilled nursing facility settings preferred
- Ability to work a 35-hour week in a SNF environment, with a schedule that includes the stated hours
- Willingness to participate in a 14-week travel assignment starting 08/03/2026
