Key responsibilities:
- Conduct thorough assessments, document findings, and monitor clinical progress in home settings.
- Administer medications and therapies per plan of care, while vigilantly identifying adverse events and escalating concerns promptly.
- Implement wound care, injections, infusion therapies, and palliative support as indicated, tailoring approaches to patient goals.
- Create, update, and communicate care plans, goals, and education across the care team and family members.
- Coordinate with physicians, case managers, and home health aides to ensure timely services and efficient scheduling.
- Provide patient and caregiver education on disease management, med safety, fall prevention, nutrition, and activity guidelines.
Professional growth and benefits:

- A structured contract designed to support consistent earnings, with the potential for contract extensions based on performance, patient needs, and organizational priorities.
- A welcome or completion bonus, as applicable, to recognize dedication and successful program milestones.
- Housing assistance options or stipends to ease transition when working in Gunnison, ensuring you have a comfortable, stable living situation during the assignment.
- Access to robust clinical development pathways, including Home Health–specific certifications, clinical mentorship, and opportunities to assume leadership roles within care coordination or field supervision as you advance.
