Responsibilities
- Administer prescribed medications and treatments under supervision, verifying doses and documenting administration.
- Monitor resident conditions, including vital signs, pain levels, and response to therapies; report changes per protocol.
- Provide direct nursing care and assistance with activities of daily living, mobility, and hygiene.
- Maintain accurate resident records in the electronic medical record, including nursing assessments and care plans.
- Collaborate with RNs and interdisciplinary team to coordinate care, detect safety risks, and implement care plans.
- Ensure infection prevention practices, wound care as directed, and safety measures to support resident well-being.
Requirements
- Active LPN license in New Hampshire
- Graduate of an accredited practical nursing program
- 2+ years of practical nursing experience in SNF or long-term care preferred
- Ability to work night shift (3x12) and participate in a travel assignment
- CPR/BLS certification preferred
- Able to work in a skilled nursing facility environment and adhere to facility protocols
