Responsibilities
- Provide skilled nursing care in the patient’s home, including assessment and ongoing monitoring of health status
- Develop, implement, and adjust individualized care plans in collaboration with the patient, family, and medical team
- Administer medications and therapies as prescribed; manage treatment administration and documentation
- Conduct patient and family education on disease process, self-care techniques, safety, and discharge planning
- Document all nursing services accurately and timely; communicate patient status to physicians and other care team members
- Coordinate with other disciplines to ensure seamless transfer of care and appropriate referrals; participate in on-call coverage as assigned
- Ensure patient safety, infection control, and adherence to clinical protocols; supervise any nursing assistants or home health aides as applicable
- Maintain compliance with licensing, regulatory, and agency requirements; complete required charting and scheduling communications
Requirements
- Active licensure to practice nursing in the applicable state (home health setting)
- Ability to work in a home-based environment; comfortable with autonomous practice and on-call expectations
- Valid driver’s license and reliable transportation
- Strong assessment, clinical judgment, and documentation skills; effective communication with patients, families, and the care team
- Prior home health or community-based nursing experience preferred
- Willingness to participate in on-call coverage as assigned
