Responsibilities
- Administer prescribed medications and treatments, and document as required in resident records.
- Monitor residents’ vital signs, assess changes in condition, and report them promptly to the supervising nurse.
- Provide direct patient care including assistance with activities of daily living (ADLs) such as bathing, grooming, dressing, and mobility.
- Maintain accurate charting, complete incident reports when needed, and communicate care plans with the interdisciplinary team.
- Adhere to safety, infection control, and medication safety protocols; ensure proper MAR and medication administration practices.
- Collaborate with other licensed staff to support continuity of care and resident comfort, dignity, and safety.
Requirements
- Active LPN license, in good standing.
- Ability to work evenings and nights; flexible to rotate shifts as needed for staffing coverage.
- Prior experience in a long-term care setting is preferred.
- Graduation from an accredited practical nursing program.
- Strong communication and teamwork skills; ability to document clearly and comply with regulatory standards.
