Responsibilities
- Provide direct patient care to residents within the scope of an LPN in a long-term care setting
- Monitor vital signs, administer medications and treatments as prescribed, and document observations
- Collaborate with RNs, physicians, and the interdisciplinary team to implement and adjust care plans
- Document care activities accurately in the electronic health record and maintain resident confidentiality
- Assist with admissions, transfers, and discharges per facility policy and procedures
- Ensure patient safety, fall prevention, infection control, and comfort; respond promptly to call lights and changes in condition
Requirements
- Active LPN license in good standing (state-specific licensure requirements apply to the assignment)
- Graduate of an approved practical nursing program
- Ability to perform 12-hour shifts in a skilled nursing/long-term care setting
- Effective communication and teamwork skills; comfortable working in a travel assignment and adapting to new policies
- Ability to maintain accurate clinical documentation and use basic electronic health record systems
