Responsibilities
- Provide direct patient care in a home hospice setting with an ER nursing background, focusing on symptom management and comfort.
- Assess patient status, monitor vital signs, respond to changes, and escalate to the supervising clinician as needed.
- Administer medications and treatments per physician orders; manage IV therapy, wound care, and other infusion-related tasks as appropriate.
- Document care activities and patient progress in the Cerner FirstNet EMR; maintain accurate and up-to-date care plans.
- Educate patients and families on disease progression, symptom management, home safety, and caregiver support.
- Collaborate with the interdisciplinary team to coordinate care, admissions, referrals, and discharge planning.
Requirements
- Active California RN license
- Basic Life Support (BLS) certification
- 1 year of RN experience in emergency or acute care; or relevant home health/hospice experience preferred
- Proficiency with Cerner FirstNet EMR or willingness to learn
- Ability to work 12-hour night shifts, including weekends as needed, in a home hospice setting
- Graduate of an accredited nursing program; BSN preferred
