Physical Therapist – Travel assignment in a long-term care setting (nursing home) in Fairmont, West Virginia, United States. Start date is 04/27/2026 for a 13-week duration. Weekly pay ranges from $2,056 to $2,193 with guaranteed hours of 35 per week. Shift is 08:00 to 16:30 (8.5 hours, daily). This role is with Healthforce Healthcare LLC on a temporary travel basis.

Responsibilities
- Conduct initial assessments and develop individual physical therapy plans of care for residents.
- Implement, monitor, and adjust therapeutic interventions to restore, improve, or maintain mobility and function.
- Document evaluations, progress notes, goals, and discharge planning in the resident’s medical record.
- Collaborate with the interprofessional team (nursing, therapy assistants, and physicians) to coordinate care and ensure continuity.
- Educate residents and families on treatment plans, safety, and home exercise programs.
- Adhere to regulatory and facility policies, including applicable documentation and infection control standards.
Requirements
- No additional license, certification, experience, or education requirements specified in the job notes.
- Ability to work in a long-term care setting and follow treatment plans within the scope of practice.
- Willingness to participate in a travel assignment and comply with assignment duration and scheduling.
