Responsibilities
- Lead coordination of patient care within the geriatric population, collaborating with multidisciplinary team to optimize outcomes.
- Act as a liaison among the patient, family, nursing staff, physicians, and care team to support an efficient treatment plan.
- Develop, implement, revise, and report the case management plan for defined patient groups from pre-admission through post-discharge.
- Monitor the patient’s clinical course, ensuring appropriate resource utilization, timely diagnostics, treatments, and procedures.
- Discuss with physicians the appropriateness of interventions, plan of care, estimated length of stay, and discharge planning.
- Coordinate transfers to higher or tertiary centers when needed and ensure continuity of information for ongoing care.
- Participate in data collection for regulatory and accreditation requirements and contribute to focus studies on selected patient cohorts.
Requirements
- Active Registered Nurse license in the state of practice (California)
- Bachelor’s Degree in Nursing (BSN) preferred
- Case management experience preferred; IP case management experience preferred
- Basic Life Support (BLS) certification required
- Minimum of two years of recent nursing experience in the field
- Ability to work in an acute care, hospital setting
