Responsibilities
- Lead coordination of patient care for a defined geriatric population from pre-admission through post-discharge, in collaboration with the care team
- Act as a liaison among patient, family, nursing staff, physicians, and other disciplines to optimize outcomes
- Serve as a consultant on specific patient items and ensure the interdisciplinary care plan aligns with the clinical course and covered services
- Monitor diagnostic tests, treatments, and procedures; discuss resource utilization, treatment plans, estimated length of stay, and discharge planning with physicians
- Focus on complex patients, frequent ED utilizers, chronic pain, substance use, and homeless patients; coordinate transfers as needed
- Provide oversight and data collection for regulatory and accreditation compliance; support focus reviews of repeated admissions when appropriate
Requirements
- Valid Registered Nurse license in California
- Basic Life Support (BLS) certification required
- RN Case Management (CM) certification required
- Minimum two years of recent nursing experience; inpatient case management experience preferred
- Education: Bachelor's Degree in Nursing (BSN) preferred
- Ability to work in an inpatient hospital setting and manage a heavy caseload
