Responsibilities
- Deliver coordinated nursing care for a patient or assigned group according to established standards of care and the nursing process.
- Supervise and direct the activities of various levels of assigned nursing staff, and coordinate care with other disciplines while using critical thinking, professional and supervisory discretion, and independent judgment.
- Assess patient conditions, monitor vital signs, administer medications and treatments per orders, and document findings in the chart.
- Provide patient and family education, support discharge planning, and coordinate with the broader home-based care team to ensure continuity of care.
- Maintain safety, comfort, and dignity for patients receiving end-of-life care, and respond to changes in condition promptly and appropriately.
- Collaborate with the CNA represented team and other professionals to optimize care delivery and patient outcomes.
Requirements
- Active Registered Nurse license in California (CA)
- Basic Life Support (BLS) certification
- Emergency department (ER) nursing experience preferred; strong clinical judgment and ability to work independently
- Ability to work 12-hour shifts in a home healthcare/hospice setting
- Willingness to travel and participate in a 13-week travel assignment
- Ability to meet the demands of a fast-paced, patient-centered environment and to collaborate effectively with a multidisciplinary team
