Responsibilities
- Administer prescribed medications and treatments per physician orders and facility protocol
- Assess residents’ conditions, monitor vital signs, and report changes to the supervising RN
- Provide direct, hands-on nursing care and assist with activities of daily living
- Document care provided, including observations, treatments, and changes in condition
- Collaborate with the interdisciplinary team to update and implement care plans
- Ensure safety and infection prevention practices are followed and maintain compliant documentation
Requirements
- Active Licensed Practical Nurse (LPN) license in New Hampshire
- Current CPR/BLS certification
- Prior experience in a long-term care or skilled nursing environment preferred
- Ability to work night shifts (3x12-hour nights) and on-site in a LTC setting
- Strong communication, observation, and documentation skills
- Eligibility to work in the United States and meet facility and regulatory onboarding requirements
