Responsibilities
- Deliver coordinated nursing care for a patient or group of patients according to established standards of care and the nursing process.
- Assess patient needs, plan and implement care, administer medications and treatments, and monitor patient responses.
- Triage and respond to acute or emergent needs, coordinating care with the interdisciplinary team while exercising clinical judgment and professional discretion.
- Supervise and direct activities of assigned nursing staff as appropriate, and communicate with physicians, therapists, and social workers to optimize patient outcomes.
- Provide symptom management and end-of-life support within a home hospice context, including education for patients and families, and assistance with discharge planning and transitions of care.
- Maintain accurate documentation, ensure infection control and patient safety practices, and comply with regulatory and specialty requirements (including trauma/ER-related protocols when applicable).
Requirements
- Active California RN license
- NCI required
- Moderate Sedation Training required
- ER nursing experience preferred; ability to work evenings; able to function in a home healthcare/hospice environment; ability to thrive in a fast-paced, dynamic setting
- Willingness to travel for a limited assignment duration and adapt to shift-based scheduling
